It is that time of year again....the time when we look back on all that we have accomplished in our PTA and feel proud.  It is also the time to win an award for those great accomplishments!
 
Go to http://www.fourthdistrictpta.org/forms.html#AWARDS and apply for the Fourth District PTA Creative Membership award, the Creative Program award, and/or the Outstanding Unit or Council award.  The application forms are easy to use....go online and open the award application and then fill in the gray fields.  Print a copy when you are finished completing the form and mail or bring it to our office--

It only takes a few minutes to fill out the application.  Make sure that you are brief but concise, and include enough details for our committee to understand your accomplishments.
